Youth in Transition Worker Program

two bears walking the larger guiding the smallerThe goal of the Youth in Transition Worker (YITW) program is to support Indigenous youth in their successful transition out of the child welfare system into adulthood.  YITWs will help youth to develop and pursue their goals, and will support them to identify, access, and navigate adult service systems relevant to their specific needs.  The YITWs will also  support youth  to connect to existing supports and resources within their communities including, housing supports, education resources, employment services and training, life skills training (e.g. financial management, household management), health and mental health services, and legal services.


Program Goals

The goal of the Youth in Transition Worker (YITW) program is to support Indigenous youth ages 16 to 24 in their successful transition out of the child welfare system into adulthood.

Your Youth in Transition Worker will provide you with support in accessing and navigating the adult services that you need in your community.


YITW can provide supports and referrals in the following areas:

  • Housing Supports
  • Education Resources
  • Employment Services & Training
  • Life Skills
  • Health and Mental Health Services
  • Legal Services

  • Self-referral
  • CAS workers
  • Family members
  • Health practitioners
  • Teachers
  • Community members
